Contractor Registration

Yearly Registration Procedure
Effective January 15, 2007

Each Contractor:
1.  Must complete a new Registration Form with current contact information,
CCB#(verification will be made with the State of Oregon), and Insurance Information.

2.  Pay yearly Registration Fee (non-refundable)
a.  $75 for company fleets of 1 to 3 vehicles                                                                                b.  $125 for company fleets of 4 or more vehicles

 

Fee must accompany the Registration form
1.  Payment by Cash or Check ONLY - made payable to Broken Top Community Association.

The complete registration form and fee can be mailed or and delivered to:

                                   PAYMENTS WILL NOT BE ACCEPTED AT THE GATE HOUSE

Starting January 15, 2007, the new requirements will take effect and any contractor that has not complied with the above requirements will be denied access to Broken Top Community.  To down load the registration form you may click on any of the "registration form" and it will load to your computer and enable you to fill it out and print off the completed form.
